The following original documents MUST be presented at the time of registration:

PROOF OF BIRTH:  Birth Certificate, Passport or Baptismal Papers
SOCIAL SECURITY CARD:
PROOF OF GUARDIANSHIP: Foster Parents: Department of Social Service Form #:DSS2999, Intro Letter from the Agency
IMMUNIZATIONS AND RECENT PHYSICAL SIGNED BY THE DOCTOR
LAST REPORT CARD/TRANSCRIPT
PROOF OF RESIDENCY:  Deed or Current Notarized Lease and two (2) of the following:
Must Be Current – Originals Only
(Within 30 days)
